Improve patient safety with an adverse event tracking system
Integrated Health System
The quality and patient safety department in an academic medical center
The current adverse event tracking software was complex and cumbersome for staff, resulting in poor utilization. As the hospital was acquiring more outpatient facilities, the department needed to ensure that adverse events were tracked across the organization.
Status:Go created a flexible, secure web-based adverse event tracking system. It included a web form that collected adverse event data from an organization employee, facilitating data submission and easing the burden on staff. The system created an adjudication process for each submission that utilized fewer departmental resources and prioritized areas for patient safety process improvement projects.
The adverse event tracking system provided managers with powerful information to improve patient outcomes and served as a catalyst for process improvement and patient safety across the organization.
- The number of adverse events reported increased by 17% over the first six months, an indication of improved system utilization
- The new system doubled the number of patient safety process improvement projects undertaken by quality department staff per quarter, from three to six
- The adverse event submission form was altered and improved four times in the first three months of operation to improve the reporting experience
